Attention all players going to the Phoenix, Arizona ODP Champions Tournament in January!

In order for you to play in the this tournament we will need the following 3 FORMS filled out from each of you NO LATER THAN 1/9/2012!

MEDICAL RELEASE, CODE OF CONDUCT, and a PUBLICATION RELEASE FORM

Those that do not have this turned in by the 9th will not be allowed to participate in the tournament. 

Please go to the following link and download the proper forms and then delivery them to the State's office, located at 2605 S. Decatur Blvd., Suite 104, Las Vegas, NV. 

Northern players can take their paperwork to Stacy's office, located at 3895 Corsair St, Suite D, Reno...sorry no faxing - we MUST have originals at check-in for the tournament.

ODP training has been set up at Kellogg Zaher Soccer Complex in Las Vegas on December 27th for the '99 & '98 GIRLS from 12:00pm - 2:00pm, and for the '97, '96' & '95 GIRLS from 2:00pm - 4:00pm, and on December 28th for the '99 & '98 GIRLS from 10:00 - 11:30am, and for the '97, '96 & '95 GIRLS from 11:30 - 1:00pm.

ALSO -  ODP training has been set up at Mackay Staduim in Reno on January 10th, and 11th, from 5:30pm - 7:00pm, all ages both boys and girls.

Additional training may be set up in Las Vegas prior to the tournament, please continue to check here for updates.
